Individual / Non-resident individual

  1. Application for registration;
  2. Passport of the signatory - the owner of the certificate for acquaintance) 1*;
  3. Copy of the passport of the signatory - the owner of the certificate 2*;
  4. The original of the taxpayer's account card (if available, for review);
  5. A copy of the taxpayer's account card (if any) 3*;

NOTES:

1* The original passport of a citizen of Ukraine or a display of a passport of a citizen of Ukraine in the application "DIIA" version 2.0 and above, a certificate of permanent (temporary) residence or a passport of a citizen of another country (refugee certificate) with a notarized translation to Ukrainian language;

2 * Copies of 1-2 pages (3-6 in the presence of marks) of the passport confirmed by the signature of the owner or copies of the document on which the signatory was identified are provided;

3 * A copy of the taxpayer's account card (if any) is certified by the owner's signature. If, due to religious beliefs, an individual has refused the registration number of the taxpayer's account card, a copy of the passport page with a note of such refusal, confirmed by the owner's signature, shall be additionally submitted. If you have a passport of a citizen of Ukraine in the form of a card containing a contactless electronic medium with a unique entry number in the Unified State Demographic Register, a copy of the taxpayer's taxpayer's registration card need not to be provided.

Legal entity

  1. Application for registration;
  2. Passport of the signatory - the owner of the certificate (for acquaintance) 1*;
  3. Copy of the passport of the signatory - the owner of the certificate 2*;
  4. The original of the taxpayer's account card (if available, for review);
  5. A copy of the taxpayer's account card (if any) 3*;
  6. A copy of the legal entity order appointing a signatory for a position 7*;
  7. Description (if any) or original of the constituent document (statute) / its certified copy (for reference) 4* 5*;
  8. A copy of the legal entity order, power of attorney, etc. to the authorized representative of the legal entity for the conclusion of transactions with third parties 6* 7* who is not specified in the Unified State Register;

NOTES:

1 * Original passport of a citizen of Ukraine or a display of passport of a citizen of Ukraine in the application "Diia" version 2.0 and above, certificates of permanent (temporary) residence or passport of a citizen of another country (refugee certificate) with a notarized translation into Ukrainian.

2 * Copies of 1-2 pages (3-6 in the presence of marks) of the passport confirmed by the signature of the owner or copies of the document according to which the signatory was identified are provided.

3 * A copy of the taxpayer's account card (if any) is confirmed by the owner's signature. If, due to religious beliefs, an individual had refused the registration number of the taxpayer's account card, a copy of the passport page with a note of such refusal, certified by the owner's signature, shall be additionally submitted. If you have a passport of a citizen of Ukraine in the form of a card containing a contactless electronic medium with a unique entry number in the Unified State Demographic Register, a copy of the taxpayer's registration card neded not to be provided.

4 * Description with a note on the code of access to the portal of electronic services of the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations, in case of amendments to the charter or creation of a legal entity after 01.01.2016. In the absence of a description, the original or a certified copy of the constituent document is submitted (for reference only). The constituent documents of a legal entity include: regulations on a state institution (branch), the charter of the legal entity (founding agreement, decision on the establishment of a legal entity acting on the basis of the model charter) or its notarized copy.

5 * Data on the state institution / organization / branch and the head of the state institution / organization / branch are checked on official sources or in the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations.

6 * A copy of the order, power of attorney, etc. to the authorized person who has the right to sign contracts on behalf of the institution, is provided in case of signing the application for registration not by the head of the institution, that is specified in the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations

7 * A copy of orders, powers of attorney for the head of the branch and employment contracts with the employer is certified by the signature of the authorized employee or the signature of the head of the legal entity, branch, employer.

8 * Copies of orders on appointment of signatories-representatives of state institutions / legal institutions are provided in case of submission of the Application for registration signed personally by the signatory and at indication in the Application for registration of signatories-representatives of legal institutions of the position.

Branches of a legal entity

  1. Application for registration;
  2. Passport of the signatory - the owner of the certificate (for acquaintance) 1*;
  3. Copy of the passport of the signatory - the ownwe of the certificate 2*;
  4. The original of the taxpayer's account card (if available, for review);
  5. A copy of the taxpayer's account card (if any) 3*;
  6. A copy of the order appointing a signatory 5*;
  7. Description (if any) or original of the constituent document / its certified copy (for reference) 4* 5*;
  8. A copy of the order, power of attorney, etc. to the authorized representative of the legal entity for the conclusion of transactions with third parties 6* 7* and who is not specified in the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations;
  9. A copy of the power of attorney for the head of the branch 7*.

7* A copy of orders, powers of attorney for the head of the branch and employment contracts with the employer is certified by the signature of the authorized employee or the signature of the head of the legal entity, branch, employer.

8* Copies of orders on appointment of signatories-representatives of state institutions / legal institutions are provided in case of submission of the Application for registration signed personally by the signatory and at indication in the Application for registration of signatories-representatives of legal institutions of the position.

Legal entity - a foreign representative office

  1. Application for registration;
  2. Passport of the signatory - the owner of the certificate (for acquaintance) 1*;
  3. Copy of the passport of the signatory - the owner of the certificate 2*;
  4. The original of the taxpayer's account card (if available, for review);
  5. A copy of the taxpayer's account card (if any) 3*;
  6. A copy of the order appointing a signatory 5*;
  7. The original of the registration certificate (for reference only) and its certified copy issued by the Ministry of Economic Development and Trade of Ukraine or the Ministry of Finance of Ukraine 4*;
  8. Copy of the certificate from the Main Department of Statistics on information from the unified state register of enterprises and organizations of Ukraine 4*;
  9. Certified copy of the power of attorney, the agreement with the head (manager) of the representative office 4*;

NOTES:

5* Copies of orders on appointment of signatories-representatives of legal institutions are provided in case of submission of the Application for registration signed personally by the signatory and at indication in the Application for registration of the position of the signatory;

Self-employed persons (notaries, lawyers, arbitration trustees, private executors, etc.)

  1. Application for registration;
  2. Passport of the signatory - the owner of the certificate (for acquaintance) 1*;
  3. Copy of the passport of the signatory - the owner of the certificate 2*;
  4. The original of the taxpayer's account card (if available, for review);
  5. Copy of the taxpayer's account card of the signatory (if any) 3*

NOTES:

1 * The original passport of a citizen of Ukraine or a display of a passport of a citizen of Ukraine in the application "Action" version 2.0 and above, a certificate of permanent (temporary) residence or a passport of a citizen of another country (refugee certificate) with a notarized translation into Ukrainian;

2 * Copies of 1-2 pages (3-6 if there are marks) of the passport certified by the signature of the owner or the document on which the signatory was identified are provided;

3 * A copy of the taxpayer's taxpayer's account card (if any) is certified by the owner's signature. If, due to religious beliefs, an individual has refused the registration number of the taxpayer's account card, a copy of the passport page with a note of such refusal, certified by the owner's signature, shall be additionally submitted. If you have a passport of a citizen of Ukraine in the form of a card containing a contactless electronic medium with a unique entry number in the Unified State Demographic Register, a copy of the taxpayer's taxpayer's registration card need not to be provided;

Self-employed employee (assistant, etc.)

  1. Application for registration;
  2. Passport of the signatory - the owner of the certificate (for acquaintance) 1*;
  3. Copy of the passport of the signatory - the owner of the certificate 2*;
  4. The original of the taxpayer's account card (if available, for review);
  5. Copy of the taxpayer's account card of the signatory (if any) 3*;
  6. A copy of the employment contract with the employer 4*;

NOTES:

1 * The original passport of a citizen of Ukraine or a display of a passport of a citizen of Ukraine in the application "Diia" version 2.0 and above, a certificate of permanent (temporary) residence or a passport of a citizen of another country (refugee certificate) with a notarized translation into Ukrainian;
